Release of Information Form

Online Release of Information Form 

To take full advantage of the program benefits, families should complete the Release of Information form. This will give students and families full access to all program opportunities that become available through My Future Fund, which may include additional funds for college and career training costs, financial education and information about planning for education after high school. 

Completing this form will allow My Future Fund staff to:

  • Check student eligibility for the additional $475 deposit, which is based on family income.
  • Share upcoming opportunities and resources with families.
  • Create My Future Fund Accounts for students who are on their district/school's directory information opt out list.

Up to 5 students can be included on one form. The form asks for the following information: 

Student's NameStudent's Date of Birth / Student's Current Grade  / Student ID Number / Student's School District

Parent or Guardian Name /  Parent or Guardian Email Parent  / Guardian Phone Number / Parent or Guardian Signature

If you do not want your child to receive a My Future Fund Account, please complete the opt out form. You can also complete the opt out form to have your child's account deactivated.
